Appointment Time
We set aside an appointment time just for you.
Our office runs on time and you will be promptly seated in the treatment room at the time of your appointment.
This process allows you the convenience to fill everything out prior to arriving for your appointment.
Online Form Security
Our online form is time-sensitive. Your session will time out due to security, so select a time you can fully complete the process.
The page may offer a print option, however due to security once the print button is selected, all previously entered information will disappear.
We provide this option for you to fill out the forms online and save you some time in our office, or if you prefer, you can fill them out in our office.
Financial Information
After your initial exam with Dr. Alan Farber, Dr. Vasiliki Batalias, or Dr. Chris Chondrogiannis, your diagnosis will be determined. Moving forward with preventive care and treatment can be a financial concern for you. We at Farber Center For Periodontics & Dental Implants understand.
Upon return to our office for your no-charge conference, the Doctor will review your recommended dental treatment plan and one of our team members will review financial options that best suit your needs. We have several payment options available to ensure you get the dental care you need.
For your convenience we accept cash, personal checks, most major credit cards, extended payment plans, and insurance. Flexible financing options are available through Proceed Finance, Care Credit, and M&T Bank. With these plans you will be able to select monthly payments that fit within your budget, turning the oral health you have always wanted into reality.
Insurance Information
We make a conscious effort to keep our fees as low as possible for your benefit. Farber Center For Periodontics & Dental Implants does not establish our fees on the basis of what your dental insurance company will pay for your needed treatment.
Our fees are based on the exceptional skill, time, judgment, professional care, and office environment provided by Dr. Alan Farber, Dr. Vasiliki Batalias, Dr. Chris Chondrogiannis, and the entire team.
We strive to continuously provide you the best care available, regardless of insurance involvement. As an added benefit for you, we may accept insurance assignment on procedures such as initial examinations, emergencies, periodontal maintenance, and surgeries.

Emergency Dental Care
Contact our office without delay. Our Periodontists will advise you as to the best course of action. While some concerns can wait until the following day, some must be treated immediately. With cases of injury, the steps you take in the first few minutes can affect long-term outcomes.
For instance, a tooth that has been knocked out of its socket can be re-implanted if handled properly and addressed within the first 30 minutes to an hour after the trauma.
Can I Prevent a Dental Emergency?
Some circumstances that create the need for emergency carelike an injury cannot be avoided, but most unplanned care can be prevented with proper home care and regularly scheduled periodontal maintenances. Maintaining the health of your teeth and gums can greatly reduce the likelihood that you will need urgent care. Our doctors can monitor your mouth to accurately predict potential trouble areas and intervene with early treatment, keeping you healthy and happy. If you have pain or an emergency situation every attempt will be made to see you that day. Seven days a week, 24-hour emergency care is available to all patients of record.
